GENTEXT/REMARKS/1.  Purpose:  To announce Office of the Secretary of Defense (OSD) directed pilot to test standardized Family Member Travel Screening (FMTS) forms, per the reference.
2.  Background.  Each Service Medical Department (Army, Air Force, and Navy) uses unique screening forms and processes to determine medical, dental, and educational suitability for dependent family members traveling to remote or OCONUS locations.  Sometimes these distinct forms and processes create challenges for Marines and their family members, particularly at joint-base and other Service installations.  In an attempt to resolve these issues, the OSD Office of Special Needs, in coordination with the Services, developed standardized FMTS forms to be tested during a DoD pilot.  The DoD Office of Management and Budget (OMB) approved use of the forms within a DoD pilot (Collection approved:  OMB 0704-0560).
3.  Situation.  The FMTS pilot will test whether the forms can be implemented within the Services’ current assignment processes.  FMTS forms will be distributed over a 90-day period, beginning 30 January 2017 and ending 30 April 2017.  The pilot process will continue until all distributed forms have been completed and returned to the losing FMTS office.  Pilot participation will be limited to Service members who are screened at a specified losing FMTS office and traveling to a specified projected duty location.
4.  Losing FMTS offices, located within military medical treatment facilities (MTF), include:  Joint Base Lewis-McChord (McChord and Madigan), Washington; Fort Carson, Colorado; Fort Jackson, South Carolina; NMC Portsmouth, Virginia; NTC San Diego, California; Naval Hospital Camp Pendleton, California.
5.  Projected gaining duty locations for the pilot include:  Germany (Baumholder, Grafenwoehr, Hohenfels, Kaiserslautern, Katterbach/Ansbach, Landstuhl, Ramstein, Stuttgart/Vaihingen, Vilseck, Wiesbaden); Mainland Japan (Atsugi, Camp Fuji, Camp Zama, Iwakuni, Misawa, Sasebo, Yokosuka, Yokota) and Okinawa, Japan.
6.  Commanders shall endorse favorably/non favorably family member travel only after receipt and review of the medical, dental and educational suitability determination located on the DD Form Test X678 (Screening Verification), Part F, Block 27a-30d (Screening Verification).
7.  For the purpose of this pilot only, commands, and Installation Personnel Administration Centers, and Administrative Units are authorized to accept the DD Form Test X678 (Screening Verification), Part F, in lieu of NAVPERS 1300/16, Part II in order to avoid duplication of work.  Marines and commands will continue to complete NAVPERS 1300/16, Parts I, III, and IV.  Marines shall submit both the DD Form Test X678 and NAVPERS 1300/16 when requesting area clearance.
8.  Marines who are not included in the pilot, will conduct overseas suitability screening using current Department of Navy forms and processes, without change.
9.  This MARADMIN will expire 1 May 2018.
