Veterans must provide a legible copy/copies of their DD-214 showing all dates of service, as well as character of service. Member copy 4 is preferred. In lieu of a DD-214, the Statement of Service (SOS) can be submitted. The SOS must include all dates of service, the approved date of discharge, terminal leave dates, and anticipated character of service (Honorable, General, etc).
