GENTEXT/REMARKS/1. FAMILY READINESS OFFICERS (FRO) ARE A FORCE MULTIPLIER AND A CONDUIT FOR COMMANDERS FOR TWO WAY COMMUNICATION BETWEEN UNITS, MARINES AND THEIR FAMILY MEMBERS. IN ORDER TO MEASURE THE IMPACT OF THE UNIT, PERSONAL, AND FAMILY READINESS PROGRAM AND IMPROVE SERVICES DELIVERED FOR THE CONTINUALLY CHANGING NEEDS OF OUR MARINES AND FAMILIES, DATA PERTAINING TO THE UNIT, PERSONAL AND FAMILY READINESS PROGRAM WILL BE COLLECTED ACROSS THE TOTAL FORCE. BEGINNING 6 AUGUST 2014, ALL FROS BOTH FUNDED, GENERAL SCHEDULE AND COLLATERAL DUTY, WILL BE REQUIRED TO TRACK AND REPORT THE DATA LISTED IN PARAGRAPH 2A REGARDING THE FOUR MISSION AREAS OF THE UNIT,PERSONAL, AND FAMILY READINESS PROGRAM (INFORMATION AND REFERRAL,OFFICIAL COMMUNICATION, VOLUNTEER MANAGEMENT, AND READINESS AND DEPLOYMENT SUPPORT). THIS DATA WILL BE COLLECTED QUARTERLY AND USED BY HEADQUARTERS MARINE CORPS FAMILY READINESS BRANCH MFZ TO SUPPORT AND ADDRESS FAMILY READINESS TRENDS AND PROGRAM REQUIREMENTS. REPORTS FROM SENIOR LEVEL FROS WILL BE DUE TO MFZ QUARTERLY IN ACCORDANCE WITH THE MARINE CORPS COMMUNITY SERVICES (MCCS)NON-APPROPRIATED FUNDS (NAF) FISCAL YEAR CYCLE (1 FEBRUARY TO 31 JANUARY). THE FIRST MONTH OF DATA FROM 6 AUGUST 2014 TO 31 AUGUST 14 WILL BE REPORTED AS A SEPARATE ACTION ITEM TO ENSURE ADMINISTRATIVE PROCESSES HAVE BEEN COMPLETED. REQUEST THAT HHQ FROS IDENTIFIED IN PARAGRAPH 4A REPORT TO HQMC THE AUGUST DATA NO LATER THAN 21 SEPT 2014. THIS REPORTING REQUIREMENT IS EXEMPT FROM REPORTS CONTROL ACCORDING TO REFERENCE (A), PART IV, PARAGRAPH 7N. EXCEPTIONS TO THIS REPORTING SCHEDULE MUST BE COORDINATED WITH MFZ.
2. DATA REQUIREMENTS:
 A. INFORMATION RESOURCE AND REFERRAL: INFORMATION (REQUESTS FOR INFORMATION) AND REFERRAL WILL IDENTIFY THE TYPE OF INFORMATION REQUESTED AND IF A REFERRAL WAS MADE REGARDING THE FOLLOWING:
(1) CRITICAL EVENTS: DEATH, DOMESTIC VIOLENCE, ILLNESS/INJURY,COUNSELING REQUEST.
(2) MISSION EVENTS: DEPLOYMENT SUPPORT, TRAINING AND/OR READINESS SUPPORT.
(3) LIFE EVENTS: CHILDCARE/SCHOOL, FOLLOW-ON INCIDENT SUPPORT, MARRIAGE/ENGAGEMENT, MOVING, PREGNANCY/NEW BABY, SEPARATION/DIVORCE.
(4) INDIVIDUAL ASSISTANCE: INDIVIDUAL AUGMENTS, EDUCATION (SERVICE MEMBER), EDUCATION (SPOUSE/FAMILY MEMBER), FINANCES, HOUSING,PAPERWORK (E.G. MILITARY IDENTIFICATION CARD, POWER OF ATTORNEY),EMPLOYMENT (SPOUSE/FAMILY MEMBER), TRICARE/MEDICAL, WELLNESS CHECK, GENERAL/LISTENING EAR.
(5) QUALITY OF LIFE: ON AND OFF INSTALLATION OPPORTUNITIES.
(6) DEMOGRAPHICS: MARINES, SPOUSES, PARENTS, AND OTHER AUTHORIZEDCONTACTS.
(7) UNIQUE INFORMATION AND REFERRAL TRENDS SPECIFIC TO UNIT/INSTALLATION.
 B. VOLUNTEER MANAGEMENT: NUMBER OF APPOINTED AND NON-APPOINTED VOLUNTEERS, NUMBER OF VOLUNTEER HOURS FOR APPOINTED VOLUNTEERS, NUMBER OF VOLUNTEER HOURS FOR NON-APPOINTED VOLUNTEERS.
 C. FAMILY READINESS EVENTS: TOTAL NUMBER OF EVENTS (E.G. FAMILY DAY EVENTS), YELLOW RIBBON REINTEGRATION PROGRAM (YRRP) EVENTS (MARFORRES ONLY), ESTIMATED ATTENDANCE, TOTAL FUNDS EXPENDITURE TO SUPPORT EVENTS.
 D. TRAINING AND OUTREACH: PROFESSIONAL MILITARY EDUCATION, COMMUNITY OUTREACH, OTHER FRO SPECIFIC TRAINING.
 E. PERSONNEL: LIST FRO VACANCIES BY UNIT, DATE OF VACANCY, NAME OF NEW FRO AND ESTIMATED DATE OF FRO REPORTING TO UNIT.
 F. TRENDS/INITIATIVES: AN OPEN TEXT FIELD THAT WILL ALLOW UNITS TO IDENTIFY SIGNIFICANT EVENTS OR DEVELOPMENTS (E.G. WEATHER EMERGENCY IMPACTS REQUIRING LARGE UNIT NOTIFICATIONS, OR BEHAVIORAL HEALTH TYPE REFERRALS THAT APPEAR TO BE EXCESSIVE IN NATURE).
3. FISCAL RESOURCES:
 A. UNIT AND FAMILY READINESS PROGRAM FUNDS (U AND FRF): BEGINNING NAF BALANCE, ALLOCATION, EXPENDITURES, ENDING NAF BALANCE, JUSTIFICATION FOR ROLLOVER FUNDS (IF APPLICABLE).
 B. INDEPENDENT DUTY FUNDS (IDF): BEGINNING NAF BALANCE, ALLOCATION,EXPENDITURES, ENDING NAF BALANCE.
 C. RESERVE FUNDS (MARFORRES ONLY): BEGINNING NAF BALANCE,ALLOCATION, EXPENDITURES, ENDING NAF BALANCE.
 D. APPROPRIATED FUNDS (APF): BEGINNING APF BALANCE, ALLOCATION,EXPENDITURES, ENDING APF BALANCE.
4. HIGHER HEADQUARTERS (HHQ) FRO:
 A. THE HHQ FROS FROM THE BELOW LISTED COMMANDS WILL COMPILE THE DATA FROM THEIR SUBORDINATE COMMANDS AND ANY OTHERS AS OUTLINED BELOW FOR THE ADMINISTRATIVE REPORTING FUNCTION OF THIS TASK: MCICOM, MCCDC, I MEF, II MEF, III MEF, MARFORCOM, MARFORPAC, MARFORRES, MARFORSOUTH, MARSOC, MCRC, MARCENT, LOGCOM. FOR THOSE UNITS WITH A HHQ NOT IDENTIFIED ABOVE, THE DATA WILL BE REPORTED AND CONSOLIDATED VIA THEIR CLOSEST REGIONAL MARINE CORPS INSTALLATION (MCI) COMMAND (E.G.MCI PACIFIC, MCI EAST, MCI WEST, MCI NATIONAL CAPITAL REGION (NCR)). THE MCICOM FRO WILL BE RESPONSIBLE ADMINISTRATIVELY FOR THE PURPOSES OF THIS REPORTING TASK TO REPORT CONSOLIDATED DATA TO MFZ FROM THOSE UNITS.
 B. A SUMMARY OF THIS DATA WILL BE REPORTED TO MFZ BY 21 SEPTEMBER 2014, 21 NOVEMBER 2014, AND 21 JANUARY 2015. AFTER JANUARY 2015,REPORTS WILL BE COLLECTED IN MAY, AUGUST, NOVEMBER, AND FEBRUARY. THE DATA WILL BE REPORTED UTILIZING THE UNIT, PERSONAL AND FAMILY READINESS DATA TOOL ON THE MFZ FRO SHAREPOINT SITE. ACCESS TO THIS SITE WILL BE MANAGED BY MFZ.
5. ALL FAMILY READINESS OFFICERS
 A. ALL FROS WILL BE RESPONSIBLE FOR SUBMITTING THEIR DATA TO THEIR HHQ A MINIMUM OF 7 DAYS PRIOR TO 21 NOVEMBER 2014 AND 21 JANUARY 2015.
 B. A SAMPLE DATA COLLECTION FORM AND THE MOST UP TO DATE DEFINITIONS OF EACH DATA FIELD IS AVAILABLE ON THE MFZ FRO SHAREPOINT SITE WITH THE INFORMATION IDENTIFIED IN PARAGRAPH 2A. UNITS MAY USE THE SAMPLE DATA FORM FOR SUBMISSION FOR STANDARDIZATION OF DATA INPUT. FROS THAT DO NOT HAVE ACCESS TO FRO SHAREPOINT SITE WILL CONTACT MFZ TO REGISTER FOR ACCOUNT ACCESS.
6. POLICY. THE REPORTING OF VOLUNTEER HOURS WILL SATISFY THE DATA COLLECTION REQUIREMENT IN ACCORDANCE WITH REFERENCE (B).
