Medical Group encourages families to schedule school physicals now

The 375th Medical Group is encouraging parents and guardians to book school physicals now by calling 256-WELL (9355).

The 375th MDG is committed to completing as many physicals as possible prior to school/sports physical day Aug. 2. Illinois law requires a school physical for children entering pre-school, kindergarten, 6th, and 9th grades. The state also requires up-to-date immunizations, eye examinations, and school physicals for all new students. Children not in the above categories do not need an annual physical unless they are involved in sports, in which case a sports physical is needed. Boy Scout and camp physicals will not be addressed on this day.

The 375th MDG has automated the Illinois school/sports physical form. The form is available on the MDG Facebook page at www.facebook.com/scottafbclinic. If unable to access or print the form, blank forms will be available at the event; however, people are highly encouraged to bring the completed form with them. Illinois High School Association and Illinois Elementary School Association forms will not be completed at this visit because the printed product that parents will receive at the end of the appointment is an acceptable substitute.

A parent or guardian must be present with the child during the physical. Dropping children off at the clinic to expedite the process is not permitted. Any issue or problem not related to the school/sports physical will be addressed by making a separate appointment with primary care managers. Expect to be in the clinic from 30 minutes to two hours.

In addition to school/sports physical day, the 375th MDG will be hosting a health fair in the Deltgen Auditorium located in the basement. Participating agencies include, but are not limited to, the Health and Wellness Center, Base Library, Illinois Poison Control, Scott Credit Union, and fingerprinting by Security Forces.
